Documents required for admission application
Applications for entrance into grades 3-8 should include items 2-4. Parents should not help students fill out item 4 except as noted in the questionnaire instructions.
Applications submitted for entrance into grades K-2 should include items 2 and 3; you may include item 4 at your discretion or if your child is interested in completing it. Parental assistance in any form (except putting words into their mouths!!) is acceptable for students applying for grades K-2. K-2 children may also draw a picture of themselves to submit with the application.
Supplementary material for the application
We appreciate receiving any information that will help us evaluate whether your child will thrive at Oasis School. Item 5 refers to additional records you can provide that will help us understand your child's strengths and needs, such as
the latest school report card, academic, behavioral or special needs evaluations done at your current or previous school, standardized test results (IQ and/or achievement), or counselor or teacher recommendations.
If we must obtain these records from the current/previous school, please ask us for a Request for Records form several weeks before the application due date, so we have adequate time to receive the records and include them in your child's admission evaluation.
If an assessment is requested for your child during the admissions process, please allow enough time to ensure that the results of the assessment are submitted by the application due date.
Negative evaluations are as useful as positive evaluations; please do not be concerned about submitting them with the application. We recognize that negative evaluations by others who have dealt with your child in educational situations may not be a true reflection of your child's abilities or personality, as difficulties often arise in situations that are not a good fit for your child's needs.
Confidentiality
All materials sent to Oasis School in support of an application for admission are confidential.
